Knee replacement surgery (total knee arthroplasty) is a life-improving procedure performed for patients whose knee joints have lost function due to advanced osteoarthritis, trauma, or rheumatic diseases. However, just as important as the surgery itself is the recovery process. Taking careful steps, sticking to the physiotherapy program, and maintaining proper nutrition greatly facilitate the patient’s return to an active lifestyle.
Day 1 – The First Step: The First Hours After Surgery
During the first few hours after surgery, the patient’s overall condition is closely monitored. Modern orthopedic clinics generally prefer epidural or general anesthesia. Within the first 24 hours, pain control is managed, and anticoagulant therapy begins.
A physiotherapist initiates early exercises to move the knee joint. At this stage, a Continuous Passive Motion (CPM) device is often used, which automatically bends and straightens the leg to prevent stiffness.
Days 2–3: First Steps Begin
From the second day after surgery, patients typically start walking with the help of a walker or crutches. Under the supervision of a physiotherapist, controlled exercises are performed.
Walks should be short but frequent — the goal is to improve blood circulation and maintain muscle strength.
Leg position and sleeping posture are also important. The leg should remain straight, and no pillow should be placed under the knee, as this can hinder full extension.
Orthopedic centers in Turkey often work in integration with modern rehabilitation units accredited by JCI (Joint Commission International). This allows international patients to complete both surgical and rehabilitation processes safely in a single center.
Weeks 1–3: Home Rehabilitation Period
Patients are usually discharged 2–3 days after surgery. The main recovery phase begins at home.
Key points to consider during this period:
- Take 15–20 minute walks daily.
- Apply ice several times a day to reduce swelling.
- Stitches are usually removed after 10–14 days.
- Keep the wound clean to prevent infection.
- Follow the physiotherapist’s recommended exercise program consistently.
At home, use pillows, chairs, and seating positions that prevent the knee from bending more than 90 degrees. This small but important detail speeds up the prosthesis adaptation process.

Months 1–3: Regaining Strength and Balance
From the first month onward, patients gradually become more active.
Under the guidance of a physiotherapist, resistance band exercises can be performed to strengthen the muscles.
Cycling, pool walking, and hydrotherapy are also recommended at this stage.
The knee usually regains its full range of motion by around the third month.
As pain decreases, patients can walk without support. However, caution should be taken when climbing stairs — always step first with the strong leg.
Months 3–6: Full Recovery Period
By this stage, the knee prosthesis is fully adapted to the body, and patients can comfortably carry out daily activities.
Flexibility, muscle strength, and balance are largely restored.
Physiotherapy sessions are typically reduced to once or twice a week.
After the sixth month, patients can usually take long walks and safely engage in light activities such as dancing or yoga.
