Ozempic vs Gastric Sleeve: Which Is Better Long-Term in 2026?
Nov 03, 2026
HEALTH,INSIGHTSOzempic vs gastric sleeve compared on long-term weight loss, 5-year cost by country, weight regain, side effects and who each option suits best.

- In a study of 44,025 real-world patients, sleeve gastrectomy kept 22.0% total weight loss at 3 years. Semaglutide kept 7.4%.
- Semaglutide works only while you take it. Most patients regain a large share of lost weight within a year of stopping.
- Over 5 years, a gastric sleeve in Turkey (€2,500–€4,000, one-time) usually costs less than continuous branded semaglutide in the US, UK or Canada.
For long-term weight loss, a gastric sleeve is more effective than Ozempic (semaglutide) for most people with a BMI of 35 or higher. Sleeve patients lose 22–25% of body weight and keep most of it off for years. Semaglutide patients typically lose 7–15% and regain weight after stopping. Semaglutide suits lower BMIs, patients who want to avoid surgery, and anyone willing to commit to lifelong treatment.
How much weight do you lose with Ozempic vs a gastric sleeve?
A gastric sleeve produces about two to three times more weight loss than semaglutide.
The largest real-world comparison followed 44,025 adults with a BMI of 35 or higher. Gastric sleeve patients lost 24.4% of total body weight at 1 year, 22.4% at 2 years and 22.0% at 3 years. Semaglutide patients lost 5.4%, 6.5%, and 7.4% over the same periods. Patients who took semaglutide continuously for a full year did better, reaching 8.8% at 3 years.
Clinical trials show higher numbers for semaglutide than real-world use. The STEP-1 trial reported about 14.9% weight loss at 68 weeks on Wegovy 2.4 mg. Real-world results are lower because many patients miss doses, use lower doses, or stop early.
A 2026 prospective study found a mean 12-month weight loss of 28.6% after sleeve surgery versus 11.3% on semaglutide.
Ozempic vs gastric sleeve: how do they compare side by side?
| Factor | Ozempic / Wegovy (semaglutide) | Gastric sleeve |
|---|---|---|
| Typical total weight loss | 7–15% | 22–29% |
| Weight loss at 3 years (real world) | 7.4% | 22.0% |
| Treatment length | Lifelong weekly injections | One operation |
| Effect after stopping | Most weight regained within 1–2 years | Permanent anatomical change |
| Eligibility | BMI ≥30, or ≥27 with a condition | BMI ≥35, or ≥30 with a condition |
| Common side effects | Nausea, constipation, vomiting | Reflux, vitamin deficiencies |
| Serious risk | Gallbladder disease, rare pancreatitis | Staple-line leak (~1%) |
| Reversible | Yes | No |
| Recovery time | None | 2–4 weeks |
Ozempic is licensed for type 2 diabetes. Wegovy, with the same active ingredient, is licensed for weight loss. Both contain semaglutide.
What happens when you stop taking Ozempic?
Weight regain after stopping semaglutide is the biggest long-term difference between the two options.
In the STEP-1 extension, patients regained about two-thirds of their lost weight within one year of stopping Wegovy. In the 2026 prospective study, nearly one-third of patients who stopped after 6 months returned to their starting weight. Blood sugar and cholesterol improvements also faded after stopping.
A gastric sleeve permanently removes about 75–80% of the stomach. This includes most of the tissue that produces ghrelin, the main hunger hormone. That is why the effect does not disappear when treatment ends.
Some sleeve patients do regain weight after 5–10 years, typically 15–25% of the weight they lost. Many now use semaglutide after surgery to manage regain instead of having revisional bariatric surgery.
Which is cheaper over 5 years: Ozempic or a gastric sleeve?
A gastric sleeve abroad is usually cheaper than 5 years of branded semaglutide.
| Country | Branded semaglutide per month | 5-year semaglutide cost | Gastric sleeve (one-time) |
|---|---|---|---|
| USA | $349 cash price ($1,349 Wegovy list) | ~$20,900 | $15,000–$25,000 |
| UK | £90–£305 (Wegovy, private) | ~£5,400–£18,300 | £9,000–£15,000 |
| Canada | CA$258–CA$291 (Ozempic) | ~CA$15,500–CA$17,500 | CA$18,000–CA$25,000 |
| Turkey | Generics expected after 2026 patent expiry | Varies | €2,500–€4,000 |
| Mexico | Varies | Varies | $4,000–$6,000 |
In the US, Novo Nordisk’s cash price for most Wegovy doses is $349 per month. The company plans to cut the US list price of Wegovy and Ozempic to $675 per month from 1 January 2027. In the UK, private Wegovy prescriptions range from about £90 to £305 per pen.
Generic semaglutide could change this. Novo Nordisk’s patent expires in early 2026 in India, Canada, China, Brazil and Turkey, and in 2031 in the UK. Generics usually cost 70–90% less than branded medicines.
A sleeve package in Turkey usually includes hospital stay, hotel and transfers. What are the side effects and risks of each option?
Semaglutide’s side effects are mostly digestive. Nausea affects roughly 40% of users, especially during dose increases. Constipation, vomiting, and diarrhoea are also common. Gallstones and pancreatitis are rare but serious.
Both options cause some muscle loss. In semaglutide trials, up to about 40% of weight lost was lean mass. Daily protein of 1.2–1.5 g per kg of ideal body weight and strength training reduce this loss, whichever option you choose.
Gastric sleeve surgery carries surgical risks. The staple-line leak rate is about 1%, and 30-day mortality is about 0.1%. The main long-term issue is acid reflux, which develops or worsens in roughly 20–30% of patients. Patients with severe reflux are usually better suited to a gastric bypass.
Sleeve patients need lifelong vitamin B12, iron, calcium, and vitamin D supplements, plus yearly blood tests.
Who should choose Ozempic and who should choose a gastric sleeve?
Semaglutide is usually the better choice if:
- Your BMI is 27–35.
- You want to avoid surgery or general anaesthesia.
- You need to lose 10–15% of body weight.
- You can afford, and are willing to take, a weekly injection long-term.
A gastric sleeve is usually the better choice if:
- Your BMI is 35 or higher, or 30 or higher with type 2 diabetes, sleep apnoea or high blood pressure.
- You need to lose 25% or more of body weight.
- You have tried GLP-1 medication and regained weight after stopping.
- You want a one-time treatment rather than lifelong medication costs.
Non-surgical alternatives sit between the two. A gastric balloon typically produces 10–15% weight loss over 6–12 months without surgery. A swallowable Elipse gastric balloon needs no endoscopy at all. Gastric Botox is a shorter-term option that produces more modest results.
Can you combine Ozempic and a gastric sleeve?
Yes. Combining them is increasingly common.
Some surgeons prescribe semaglutide before surgery to reduce liver size and lower surgical risk in patients with a very high BMI. Others prescribe it 2–5 years after surgery to treat weight regain or a weight-loss plateau. Massive weight loss from either option can leave loose skin. Many patients later consider a body lift or tummy tuck once their weight has been stable for 6–12 months.
For long-term results, a gastric sleeve outperforms Ozempic for most people with a BMI of 35 or higher. It delivers about 22% weight loss at 3 years versus 7–9% with semaglutide, and its effect does not depend on ongoing medication. Semaglutide is effective, reversible, and surgery-free, but it works only while you keep taking it. Over 5 years, a sleeve abroad often costs less than branded semaglutide at home. Wupdoc connects you with verified bariatric specialists worldwide, so you can compare a one-time sleeve with long-term medication and decide based on real costs and results.
